Applications - Health & Temperament Agreement

1. I understand that I am solely responsible for any harm caused by my dog(s) while attending day/playcare and/or sleepovers. I hereby release It's A Dog's Life day/playcare, its owners, agents, officers, directors, employees and volunteers from ordinary negligence liability for injuries to myself, my dog(s) or any other property of mine, which arises in any of the services provided by or as a consequence of my association with It's A Dog's Life day/playcare. I understand there are certain risks involved with dog ownership, training and care including, but not limited to fights, bites, and the transmission of disease. By my signing below, I acknowledge and accept responsibility for these and all other risks and release It's A Dog's Life day/playcare of any liability arising out of ordinary negligence.

2. I further understand and agree that in admitting my dog(s), It's A Dog's Life day/playcare has relied on my representation that my dog(s) is/are in good health and have not harmed or shown aggression or threatening behavior towards any person and/or any other dog.

3. I further understand and agree that It's A Dog's Life day/playcare and their staff and/or volunteers, will not be liable for any problems that develop. It's A Dog's Life day/playcare assumes no liability for loss or damage from medicinal applications, disease, sickness, death, running away, theft, fire, injury to persons, other pets or property by my dog(s) or other unavoidable causes, due diligence and care having been exercised.

4. I also assume full financial responsibility for any and all expenses, including veterinary care, involved with my association with It's A Dog's Life day/playcare, including but not limited to any harm caused by my dog(s) while attending day/playcare and/or sleepovers. I further agree to indemnify and hold It's A Dog's Life day/playcare harmless against any claims arising out of my dog(s) attendance and participation at It's A Dog's Life day/playcare, including but not limited to, all costs, attorney fees, expenses and liabilities in connection therewith.

5. I further understand and agree that any problems that develop with my dog(s) will be treated as deemed best by the owner, staff and/or volunteers of It's A Dog's Life day/playcare in their sole discretion. I authorize veterinary care, including medical and surgical treatment if needed, to provide for the safety and comfort of my dog(s) and agree to assume full financial responsibility for any fees, which may result. It's A Dog's Life day/playcare reserves the right to deny entry or remove any dog(s) from the premises at their discretion.

I certify that I have read and fully understand the Rules & Regulations, Health & Temperament Agreement and Bill-of-Health set forth on the preceding pages. I agree to abide by all rules and regulations and accept all terms, conditions and statements of these agreements.
